The following links are forms and information for participation in athletic events in Brimfield CUSD #309. Forms can be printed, filled out, and returned to the coach, athletic director, or office.

PLEASE NOTE:

The pre-participation athlete and parent packet must be completely filled out by parents and student athletes in Skyward before an athlete can participate in any practices or games.

A current sports physical must also be on file with the school district before an athlete may participate in any practices or games. The physical form is located below, but most doctor offices  and clinics will have these forms on hand. (To be filled out by your physician and returned to school, most doctor's offices or clinics will already have this form on file)
