School Exam/Immunization Requirements

SUMMARY OF SCHOOL REQUIREMENTS

FOR USE BY SCHOOL STAFF

Pre-K

  • Physical Exam
  • Lead Test/Screening
  • Immunizations                    

     4 or more doses DTaP (booster after 4th birthday)

     3 or more doses Polio (booster after 4th birthday)

     2 doses MMR (first one must be after first birthday)

     1 dose Varicella (Chicken Pox)

     1 or more Hib doses

     3 Hepatitis B doses

 Kindergarten:

  • Physical exam
  • Dental Exam
  • Eye Exam
  • Lead Test/Screening
  • Immunizations                    

     4 or more doses DTaP (booster after 4th birthday)

     3 or more doses Polio (booster after 4th birthday)

     2 doses MMR (first one must be after first birthday)

     1 dose Varicella (Chicken Pox)

                   *Second dose of Varicella at age 4-6 years recommended but not

                    required

2nd Grade

  • Dental Exam

 5th Grade

  • Immunizations

     Record of having 3 doses of Hepatitis B vaccine (normally given in infancy)

 6th Grade

  • Dental Exam
  • Physical Exam

 9th Grade

  • Physical Exam
  • Immunizations

      Td Booster (Tdap recommended) if 10 years since last tetanus booster

 Recommended shots at 11-12 years of age:

  • Meningococcal Meningitis
  • HPV (girls only)
  • Tdap (Tdap given at this age will meet requirement for Td booster at 9th grade)
